Let's Talk
Whether you're a clinic exploring a partnership or a patient with questions, we'd love to hear from you.
For Patients & Families
Chronic Care That Feels Personal
You see your doctor a few times a year. We're the care team that checks in every month in between.
What Is Chronic Care Management (CCM)?
If you're managing two or more ongoing health conditions — like diabetes, high blood pressure, or heart disease — Chronic Care Management gives you a dedicated care team that checks in by phone every month between doctor visits, following the care plan your physician has set for you.
- Better medication & care plan adherence
- Stronger health outcomes through consistent monitoring
- 24/7 access to a clinical support line
- Fewer avoidable emergency room visits
Eligibility: CCM is available to Medicare patients managing two or more chronic conditions. Ask your provider if you qualify.
What Is Remote Patient Monitoring (RPM)?
RPM uses simple, connected devices — like a blood pressure cuff or pulse oximeter — to send your vital signs to your care team automatically, so changes in your health can be caught early, often before you'd notice symptoms yourself.
- Real-time tracking of vitals from home
- Early detection of warning signs
- No extra trips to the clinic required
- Care team alerted automatically to significant changes
Eligibility: RPM is available to patients with a wide range of chronic or acute conditions — there's no minimum number of diagnoses required.
What Is Behavioral Health Integration (BHI)?
Mental and emotional health are part of chronic care too. BHI brings behavioral health support directly into your primary care team, including care planning and access to psychiatric consultation when it's needed — no separate referral required.
- Behavioral health support built into your existing care team
- Coordinated care planning
- Access to psychiatric consultation
- Ongoing check-ins, not just a one-time referral
How It Works
From Referral to Ongoing Care
- 1Your provider enrolls you — CCM, RPM, and BHI all start with a referral from your physician.
- 2You hear from your care team monthly — a phone call, and/or automatic readings from your monitoring device.
- 3Your care plan stays on track — medication reminders, symptom check-ins, and coordination with your doctor.
- 4Help is always available — a clinical support line is there when you need it, 24/7.

1. Overview
This Privacy Policy explains how Bessa Health, LLC ("Bessa Health," "we," "us") handles information collected through BessaHealth.com (the "Site"). It covers the Site only — it does not cover the clinical records, care plans, or health information your care team keeps as part of the Chronic Care Management, Remote Patient Monitoring, or Behavioral Health Integration services delivered through your provider.
2. This Site Is Not a Patient Portal
BessaHealth.com is an informational and marketing website. It is not used to deliver care, store medical records, or transmit Protected Health Information (PHI) as defined under HIPAA. Please do not submit PHI — such as diagnoses, medications, or treatment details — through any form on this Site. If you have a care-related question, contact your provider or care team directly.
3. Information We Collect
We collect information in two ways:
Information you provide. When you use our contact form, this may include your name, email address, phone number, the option that best describes you (patient, clinic or provider, prospective employee, or other), and the message you send us.
Information collected automatically. Like most websites, we may collect standard technical data — such as browser type, device type, pages visited, and approximate location derived from IP address — to understand how the Site is used and to keep it secure.
4. How We Use Information
We use the information above to respond to inquiries, route messages to the right team (care partnerships, careers, or general support), maintain and improve the Site, and meet legal or regulatory obligations. We do not use information submitted through this Site for automated clinical decision-making, and we do not sell personal information to third parties.
5. Cookies & Similar Technologies
The Site may use a limited set of cookies or similar technologies to remember basic preferences and to understand aggregate traffic patterns. You can control or disable cookies through your browser settings; doing so may affect some Site functionality but will not affect your ability to reach us by email.
6. How We Share Information
We may share information with service providers who help us operate the Site (such as hosting or email delivery), when required by law or to protect our legal rights, or in connection with a merger, acquisition, or sale of assets. We do not share the contents of your contact form submission with unaffiliated third parties for their own marketing purposes.
7. Data Retention
We retain contact form submissions only as long as needed to respond to your inquiry and to maintain reasonable business records, after which they are deleted or de-identified.
8. Your Choices
You may ask us to delete personal information you've submitted through this Site, or ask what information we hold about you, by emailing info@BessaHealth.com. Depending on your state of residence, you may have additional rights under applicable privacy law; we will honor valid requests consistent with those laws.

This Site is intended for a general adult audience — patients, caregivers, clinic staff, and prospective employees — and is not directed to children. We do not knowingly collect personal information from children through this Site.
10. Security
We use reasonable administrative and technical safeguards to protect information submitted through the Site. No method of transmission over the internet is completely secure, which is one reason we ask that PHI never be submitted here.
